Advance Directives

When you enroll in MedicareBlue PPO, you will receive information about advance directives. The Federal government requires us to share this information with our MedicareBlue PPO members in an effort to educate people about this important health care treatment communication vehicle.

Advance directives provide written documentation and instructions to health care providers on how you wish to be cared for in the event of an accident, terminal illness, or other major health care event. It is important to discuss your wishes with your family, loved ones and health care providers.

If you decide to complete an advance directive, you should share copies with family members and your doctor(s).
