MedicareBlue PPO
Heart Failure Program
MedicareBlue PPO offers a special program for members with heart failure. This program is designed to help members manage their health and improve their overall quality of life. You will work with a MedicareBlue PPO clinician who will provide tips and support for meeting the demands of living with heart failure. They will cover many topics such as weight monitoring, medications symptom management, exercise, blood pressure, low-salt diet, and rest.
Getting Started in the Program
It starts with an introductory letter for eligible members. Then you’ll receive a confidential welcome call from a MedicareBlue PPO clinician. If you agree to participate in this voluntary program, you’ll receive one-on-one support to help you better manage your health-related needs.
Your clinician will:
- Talk with you about your condition
- Review your medications and current treatment
- Talk about your best strategies, set goals and create an action plan
- Help you understand your doctor’s recommendations and support the plan of care
- Connect you to other helpful programs, as needed
- Answer any questions or concerns you may have
You and Your Doctor
Our MedicareBlue PPO clinician may keep your doctor informed of your program progress to ensure coordination of your care. Members who participate in the program may find that the information and support they receive helps them take a more active role in their health care, make more informed decisions, and strengthen their relationship with their health care provider.
The MedicareBlue PPO Heart Failure Program is based on the American College of Cardiology/American Heart Association’s (ACC/AHA) 2005 guideline for the Diagnosis and Management of Chronic Heart Failure in the Adult.
Participation in the MedicareBlue PPO Heart Failure program is voluntary and is available at no cost to you.
