MedicareBlue PPO

Health Risk Assessment Survey

When you enroll in MedicareBlue PPO, you will receive a health risk assessment survey form by mail. This survey measures general health indicators and identifies how our MedicareBlue PPO nurses may be able to help you by coordinating access to the appropriate services, or help you understand your medical condition and how to stay as healthy and active as possible. Participation in the Health Risk Assessment is voluntary and is available at no cost to you.
