Order ID Card - Group Products

Information for Individual Members

This page applies to you if you receive your Group MedicareBlue PPO or Group MedicareBlue Rx plan through your employer group or union. If you purchased your Plan on your own, refer to Order ID Card.

Do you need a replacement ID card? Follow the instructions below to receive the appropriate ID card you need. Your ID card shows information important to your health insurance coverage or prescription drug plan. You don't need a new ID card every year. If you change plans, we will send you a new ID card.
