Eligibility Requirements

You must:

• Be entitled to Medicare Part A and enrolled in Medicare Part B.
• Continue to pay your Medicare Part B premiums (and Part A, if applicable) if not otherwise paid for by Medicaid or another third-party.
• Be a resident of one of the following states: Iowa, Minnesota, Montana, Nebraska, North Dakota, South Dakota or Wyoming.
• Not have ESRD (end stage renal disease), unless you are converting from other Blue Cross and Blue Shield coverage during your initial enrollment period, or if your current Medicare Advantage plan stops providing coverage in your area. You may also be eligible to enroll if you do not need regular dialysis, or have had a successful kidney transplant. In this instance, you must attach a note or records from your doctor showing you do not need dialysis or have had a successful kidney transplant.

Note: If you currently have health coverage from an employer or union, joining MedicareBlue PPO may change how your current coverage works. Read the communications your employer or union sends you, visit their Web site, or contact the office listed in their communications or the benefits administrator of that plan if you have questions about your coverage.
