Disease Management for Heart Failure
Heart Failure is the only disease management program currently being offered for MedicareBlue PPO. The MedicareBlue PPO Heart Failure Program is based on the American College of Cardiology/American Heart Association’s (ACC/AHA) 2005 guideline for the Diagnosis and Management of Chronic Heart Failure in the Adult.
